A free organisational template for specialists and families

Goals and observations for the next session

Keep a specialist-selected goal separate from what happened during a particular activity. This sheet preserves the context and helps prepare the next step.

How to keep a useful working note

  1. Write the goal in your professional terms and record when it was selected. This template does not choose a goal for you.
  2. Describe the observation as a specific fact. Include the material used and the support that was needed.
  3. Keep family questions and points to clarify in a separate column. A short observation is not a diagnosis.
  4. Before the next session, select the next action. The specialist evaluates the outcome and decides whether to change the plan.

An example using synthetic data

Organisational goal: the family understands the assigned practice instructions. Observation: the adult opened the material but stopped at the second step. Context: home practice after the session. Question: should we open the example first? Next step: demonstrate the sequence at the next appointment.

Keep goals and history in the child’s record

In Cadenivo, goals and history stay with the child’s record. Return to the previous activity and family response when preparing for the next session.
